Registration for 2012

To register your student for Marching Band, you will need to download and complete several registration forms located on the subtab in this section of the website.  Completed forms may be turned in at the Volunteer Fair on Saturday May 19 – 1:00 PM – 3:00 PM. For your convenience, notaries will be available at the Fair to assist you in completion of the Health Form.  PLEASE NOTE: Two copies of the Health Form are required and they must be NOTARIZED!   Thereafter, forms may be turned into Mr. Saucedo or mailed to him at the following address: CHS Performing Arts Department, Carmel High School, 520 E. Registration will not be considered complete until you have signed up to assist with one of the two fundraisers (Car Wash or Community Night)  and at least one other volunteer opportunity that assists the Marching Band.   Online registration for volunteer commitments for the 2012 Marching Season is available at:  CMB VOLUNTEER.

If you have not had an opportunity to sign up to volunteer, please contact Kim Ware for sign up for Guard Activities and Lisa Harbour for all other activities.
